Frozen Oocyte (Egg) Replacement Cycle (FORC) with Donor Eggs and Surrogate

Hello,

Before your doctor can prescribe a frozen oocyte(egg) replacement cycle (FORC) with donor eggs and a surrogate, you must first complete tests and meet with our psychologist for counselling and evaluation. 

Only tests performed at the MUHC will be automatically sent to us. It is your responsibility to ensure that results for tests performed outside the MUHC reach us before your telephone consultation with the physician.

Test results should be faxed to 514 843-1496. Please do not call for your test results; the staff is not permitted to give results over the telephone.

1. Tests for the Intended Parents

Call 514 843-1650 to schedule an appointment.

Bring the blood test requisition to your appointment. It is attached to the email you received.

The baseline blood tests are done at the:

Fasting is not required.

As per Health Canada Guidelines, serology blood test results are valid for 6 months.

This test cannot be done when menstruating.

Bring the urine test requisition to your appointment. It is attached to the email you received.

If you are an out-of-town patient, you can do this test in your area.

As per Health Canada Guidelines, test results for Chlamydia/Gonorrhea are valid for 6 months.

Instructions:

  • The sample must be produced at home.

  • Obtain a sterile specimen container from the pharmacy.

  • Avoid ejaculation for 2 to 3 days maximum before the semen analysis.

  • Before producing a sample, wash and thoroughly rinse the penis and groin area from all soap residues. Wash and rinse hands before masturbation.

  • Keep the semen sample as close to body temperature as possible.

  • Come alone to the clinic within 60 minutes of the time of ejaculation.

  • At the time of the appointment, you will be asked to provide your RAMQ Medicare card as well as the hospital card. A valid photo ID is required to confirm your identification.

 

2. Counselling and Evaluation

All patients involved in third-party reproduction are required to schedule a telephone consultation with our psychologist for counselling and evaluation.

If you are in a couple, both partners must be present on the call.

4. Legal Counsel

As intended parents, you should consult with your own lawyer to ensure that you are aware of the legal issues surrounding surrogacy.  You must provide a letter from the lawyer confirming that you have sought counsel regarding the surrogacy arrangement. Please email the lawyer's letter to fertility[at]muhc.mcgill.ca and add your physician's name in the subject line. For example, SUBJECT: Lawyer's Letter Atten. Dr. ______________.

 


Steps 1-4 must be completed before your telephone consultation with the physician.

 

5. Telephone Consultation with your Physician

Once you have completed all the above steps, schedule a telephone consultation with your physician. This follow-up appointment is for both parties (i.e., Surrogate and Intended Parents).

6. Payment

Once you have the medical order and are ready to proceed, call 514 934-1934 ext. 36834 to pay for treatment. We accept VISA, Mastercard, and American Express.

Once you have the medical order, a nurse will review your file and contact you to coordinate your treatment and schedule your calendar appointment.

Please complete, sign and bring to your calendar appointment with the nurse:

 

Note: Treatment will be cancelled if the blood test results for all parties (intended parents and surrogate) are not valid or if the consent forms have not been received.
