Pre-Treatment Steps for the Potential Surrogate

 

Hello,

Thank you for your interest in becoming a surrogate. To learn more about the process, please read our surrogacy booklet.

As part of the screening process, potential surrogates are required to complete a series of tests. 

Only tests performed at the MUHC will be automatically sent to us. It is your responsibility to ensure that results for tests performed outside the MUHC reach us before your telephone consultation with the physician.

Test results should be faxed to 514 843-1496. Please do not call for your test results; the staff is not permitted to give results over the telephone.

1. Tests

Call 514 843-1650 on day 1 of your cycle to schedule an appointment for a baseline vaginal ultrasound to be done on Day 2 to 5 of your cycle.

The baseline vaginal ultrasound is performed at the:

Glen Site
1001 Decarie Blvd.
Obstetrics and Gynecology Ultrasound
6th floor of Block C, Room C 06.4351. 

If an appointment is not available, wait for your next cycle.

If you are an out-of-town patient, you can do the ultrasound in your area. In this case, use the requisition attached to the email.

Call 514 843-1650 to schedule an appointment.

The baseline tests can be done on the same day as your baseline ultrasound appointment.

Bring the baseline blood requisition to your appointment. It is attached to the email you received.

The baseline blood tests are done at the:

Glen Site Blood Test Centre
1001 Decarie Blvd.
2nd floor of Block D (south side) Room D 02.2307

Fasting is not required.

* As per Health Canada Guidelines, serology blood test results are valid for 6 months.

Call 514 843-1650 to request an appointment.

This test must be done when you are not menstruating.

Bring the urine test requisition to your appointment. It is attached to the email you received.

If you are an out-of-town patient, you can do this test in your area.

* As per Health Canada Guidelines, serology blood test results are valid for 6 months.

The Pap test must be done when you are not menstruating.

Bring the Pap test requisition to your appointment. It is attached to the email you received.

The Pap test can be done at our Clinic by appointment; however, there is a wait time for results. For faster results, this test can be done in a private clinic for a fee.

If you are an out-of-town patient, you can do this test in your area.

 

 

2. Tests for Partner (if applicable)

Call 514 843-1650 to schedule an appointment.

Bring the baseline blood requisition to your appointment. It is attached to the email you received.

Both partners can do their blood tests on the same day at the Glen Site:

3. Treatment Consent Forms

You will need to print, complete, and sign the treatment consent and COVID-19 consent forms.  Please bring the consent forms signed by all parties to your calendar appointment.

4. Counselling and Evaluation

All patients involved in third-party reproduction are required to schedule a telephone consultation with our psychologist for counselling and evaluation.

If you are in a couple, both partners must be present on the call.

 

5. Legal Counsel

As a gestational carrier, you should consult with your own lawyer to ensure that you are aware of the legal issues surrounding surrogacy.  You must provide a letter from the lawyer confirming that you have sought counsel regarding the surrogacy arrangement. Please email the lawyer's letter to fertility[at]muhc.mcgill.ca and add your physician's name in the subject line. For example, SUBJECT: Lawyer's Letter Atten. Dr. ______________.

 

6. Telephone Consultation with your Physician

Once you have completed all the above steps, schedule a telephone consultation with your physician. This appointment is for both parties (i.e., Gestational Carrier and Commissioning Couple) and is to review all test results and your compatibility with the intended parent(s). If all test results are available and no other tests are required, a medical order will be added to your file.

Once you have the medical order, a nurse will review your file and contact you to coordinate your treatment and schedule your calendar appointment.
