Egg Donor Screening

 

Hello,

As part of the donor screening process, potential egg donors are required to complete a series of tests. 

Only tests performed at the MUHC will be automatically sent to us. It is your responsibility to ensure that results for tests performed outside the MUHC reach us before your telephone consultation with the physician.

Test results should be faxed to 514 843-1496. Please do not call for your test results; the staff is not permitted to give results over the telephone.

1. Tests

Call 514 843-1650 on day 1 of your cycle to schedule an appointment for a baseline vaginal ultrasound to be done on Day 2 to 5 of your cycle.

The baseline vaginal ultrasound is performed at the:

Glen Site
1001 Decarie Blvd.
Obstetrics and Gynecology Ultrasound
6th floor of Block C, Room C 06.4351. 

If an appointment is not available, wait for your next cycle.

If you are an out-of-town patient, you can do the ultrasound in your area. In this case, use the requisition attached to the email.

Call 514 843-1650 to schedule an appointment.

The baseline tests can be done on the same day as your baseline ultrasound appointment.

Bring the baseline blood requisition to your appointment. It is attached to the email you received.

The baseline blood tests are done at the:

Glen Site Blood Test Centre
1001 Decarie Blvd.
2nd floor of Block D (south side) Room D 02.2307

Fasting is not required.

* As per Health Canada Guidelines, serology blood test results are valid for 6 months.

Call 514 843-1650 to request an appointment.

This test must be done when you are not menstruating.

Bring the urine test requisition to your appointment. It is attached to the email you received.

If you are an out-of-town patient, you can do this test in your area.

As per Health Canada Guidelines, Chlamydia/Gonnorrhea test results are valid for 6 months.

Call 514 843-1650 to request an appointment.

The PAP test must be done when you are not menstruating.

Bring the PAP test requisition to your appointment. It is attached to the email you received.

The PAP test can be done at our Clinic by appointment; however, there is a wait time for results. For faster results, this test can be done in a private clinic for a fee.

If you are an out-of-town patient, you can do this test in your area.

If you are doing the urine and/or PAP test at our Clinic, the physical exam can be done on the same day.

If you are an out-of-town patient or wish to do this exam elsewhere, please use the Physical Exam Form.

    2. Donor Intake Form

    Please complete the donor intake form and return it to the clinic.

    If you are doing the urine/PAP test, or physical examination at our Clinic, bring the completed donor intake form on the day of your appointment.

     

    3. Treatment Consent Forms

    You will need to complete, sign and bring to the Centre the following treatment consent forms. If you are coming to the Centre for an appointment, please bring the signed forms to your appointment.

     

    4. Counselling and Evaluation

    Call 514 843-1650 to schedule the appointment.

    All patients involved in third-party reproduction are required to schedule a telephone consultation with our psychologist.

    Partners must be present on the call. Partners who cannot be present must submit a letter acknowledging awareness of the donation.

     

    5. Telephone Consultation with your Physician

    Once you have completed all the above tests, call 514 843-1650 to schedule a telephone consultation with your doctor to review the results.

    If you are determined suitable for donation, a medical order will be added to your file.

     

    6. Calendar

    Once all the above steps are completed, a nurse will call you to plan your treatment.
