Frozen Embryo Replacement Cycle (FERC)

Public Funding of In Vitro Fertilization (IVF) – Effective November 15, 2021

If you intend to participate in a fertility project covered by RAMQ, each participant involved in the project (you and your partner (if applicable)) must complete and send the RAMQ Undertaking Form to the following email: [email protected]  Please write UNDERTAKING FORM in the subject line of the email and send only the RAMQ Undertaking Form to this address.

Until we have received the RAMQ Undertaking Form for each partner, we cannot confirm your undertaking to the RAMQ. Therefore, please send the RAMQ Undertaking Form(s) at your earliest convenience to avoid delaying your treatment.

If you have questions regarding eligibility or insured services, please contact the RAMQ.

If you are eligible for RAMQ coverage, you may choose one of the following options:

• Pay for this frozen cycle (FERC) and save for later the possibility of doing an IVF (fresh) cycle covered by RAMQ.
• Instead of the frozen cycle (FERC), do an IVF (fresh) cycle covered by RAMQ and if not pregnant continue with RAMQ covered frozen cycles (FERC) until the age limit (42). If you choose this option, you will need to make an appointment with your physician for a new medical order.
• Use the RAMQ coverage for this frozen cycle (FERC) and forfeit your eligibility for a future RAMQ covered IVF (fresh) cycle.

Payment

If you are not eligible for the RAMQ coverage or choose to pay for treatment, please call 514 843-1650 ext. 36834 to make your payment. We accept VISA, MasterCard, and American Express. 

All patients, whether paying or covered by RAMQ, will receive a call from a nurse to coordinate treatment and schedule a calendar appointment.
