New patients
All patients visiting the MUHC for the first time must go to the Admitting Office to register. Please bring your valid Quebec health insurance card.
If you do not have a Quebec health insurance card, please bring at least one of the following:
- valid health insurance card from another Canadian province or territory
- piece of government-issued photo ID (driver’s license, passport, etc.)
- proof of private health insurance coverage
- document proving refugee status
The MUHC no longer issues hospital cards. Effective August 28, 2025, we only require your health insurance card. Make sure it has not expired.
Returning patients
If there has been a change in any of your personal information (address, home or cell phone number, email address, etc.), you must go to the Admitting Office before visiting your clinic to update your file. Do this at least 30 minutes before your scheduled appointment.
