The SSPPCC project is now fully rolled out at the MUHC

We are pleased to announce that the rollout of the Early Collaborative Care and Discharge Planning (SSPPCC: in French the "Suivi des séjours et planification précoce et conjointe des congés) project is now complete. Implementation began in spring 2023 following a ministerial request. The project was adapted to the institutional reality of the MUHC in order to integrate the requirements in a realistic and sustainable manner, building on the strengths and expertise of our clinical teams. As Catherine Jetté-Robert, Project Manager at the Organizational Project Management Office (OPMO), points out, “This approach certainly helped to promote its adoption.” 

The SSPPCC aims to improve care coordination and optimize discharge planning from the moment the patient arrives, based on an estimated discharge date. This approach relies on the involvement of multidisciplinary team members in partnership with patients, their loved ones, and network partners, in a context where initiatives and service offerings are constantly changing. 

“The various strategies enabled the successful integration of the project’s different objectives,” emphasized Johnny Memettre, Continuous Improvement Advisor. 

“The project team witnessed the excellent work accomplished by the clinical teams, despite numerous constraints. Their commitment and adaptability are remarkable,” highlighted Marie-Ève Leblanc, Nursing Practice Consultant, Nursing Directorate, and Diana Valentini, Coordinator, Multidisciplinary Services.

For example, at the Glen, on D8, the team's efforts have so far led to a significant and sustained decrease in the length of stay on their unit.

Noël Dowell, Manager of the D8 team, noted, "Since implementing the SSPPCC program there has been increased detection and communication of issues that may affect a timely discharge, and a culture of heightened accountability has developed amongst multidisciplinary team members." 

Although efforts continue in each unit to maintain this momentum and further improve the patient experience, the impressive work accomplished by everyone deserves to be celebrated.

The management committee, led by Nancy Plaisir, will now oversee the SSPPCC in order to support teams in improving care pathway flow and continuously enhancing the patient experience.
