Patients:
You need to be referred by a physician to get an appointment in the Rheumatology Division at the MUHC. Once your referral has been received and triaged, you will be contacted to set up an appointment as soon as possible.
Referring physicians:
Please use the Rheumatology Referral & Triage Form to ensure the best possible delay in giving the patients their appointments and consult the included information on expectant wait time guidelines. Note that you can always discuss any particular case with the rheumatologist on call if you feel a patient needs to be seen earlier.
Please include with the referral the patient’s MGH/RVH # if applicable, as well as a daytime phone number where the patient can be reached. The patient will be contacted to set up an appointment as soon as possible once the referral has been triaged.
You can send the referral form directly to the Division of Rheumatology by fax or, if you work at the MUHC, by internal mail.
