You now need to e-mail the coordinators of the screening program and you will be e-mailed back within the next business day with the appropriate form and instructions to have your blood drawn. Please note that you will be asked to fill in some parts of the requisition form and to go to the Test Centre of the McGill University Health Center Glen site to make your hospital card and to have your blood drawn. Only people covered by Quebec Medicare can have access to these tests through this website.
Please send an e-mail to [email protected] that includes the following demographic information:
- full name;
- date of birth;
- Medicare number and expiration date;
- address and phone numbers (please provide us with the best phone number to reach you at once the results have been received);
- full name of parents.
In addition to the above, if you or your partner is pregnant, you must include the following information:
- the date of your or your partner's last menstrual period (LMP)
- the date of expected delivery
- your partner's demographic information (see above)
