The Quebec Medically Assisted Reproduction Program

If you intend to participate in a fertility project covered by RAMQ, each participant involved in the project (you and your partner (if applicable)) must complete and send the RAMQ Undertaking Form to the following email: [email protected]  Please write UNDERTAKING FORM in the subject line of the email and send only the undertaking form to this address.

Until we have received the RAMQ Undertaking Form for each partner, we cannot confirm your undertaking to the RAMQ. Therefore, please send the RAMQ Undertaking Form(s) at your earliest convenience to avoid delaying your treatment.

Payment

If you are not eligible for the RAMQ coverage, visit the Pricing and Policies section of our website for more information. We accept VISA, MasterCard, and American Express. 

All patients, whether paying or covered by RAMQ, will receive a call from a nurse to coordinate treatment and schedule a calendar appointment.

Blood Tests

Blood tests for you and your partner (Hepatitis B, Hepatitis C, Hep B surface antibody (Anti‐HBs), HIV, and Syphilis) must be valid before you can start treatment. Blood tests are valid for 12 months. If needed, you will be asked to repeat the blood tests.

If you are doing blood tests, call 514 843-1650 to schedule an appointment.
