If after reading the documents you wish to proceed with treatment, follow these steps:
- Call 514 843-1650 ext. 36834 to pay for treatment. We accept Visa, MasterCard, and American Express. Once you have made your payment, a nurse will contact you to coordinate your treatment and schedule your calendar appointment.
- Complete, sign and bring to the calendar appointment:
Blood Tests
Blood tests for you and your partner (Hepatitis B, Hepatitis C, Hep B surface antibody (Anti‐HBs), HIV, and Syphilis) must be valid before you can start treatment. Blood tests are valid for 12 months. If needed, you will be asked to repeat the blood tests.
