The Palliative Care Day Hospital team provides an outpatient service that assesses:
- Patients' physical, psychological and social needs
- Facilitates tests and interventions
- Offers excellent symptom management and end-of-life care planning
- Works with CLSCs to ensure services are in place to allow patients to remain at home as long as possible
