Question: Where can I find a request form in order to get a copy, or a section of my medical record? Do I need to make the request in person? If so, where would I go?
Answer: You will need to make your request in person at the Access to Health Information Office of the site where you have received services from. Please have on hand a form of government issued identification (RAMQ card, driver’s license, passport, etc.).

- Montreal General Hospital: L6.120 (Cedar main entrance)
- Montreal Children’s Hospital: CRC.7103 (Larry and Cookie Rossy Promenade)
- Montreal Neurological Hospital and Institute: 163 (1rst floor)
- Lachine Hospital: 2C3
You can view your medical record in person free of charge. If you wish to obtain a copy, fees may be charged depending on the length of your file.
