How to book an appointment
New patients will only be provided an appointment if they are referred by another physician.
If you are in possession of a consultation request from your doctor to see a vascular surgeon or for vascular lab studies, please fax the consultation and you will be contacted for your appointment (514 843-1730).
IMPORTANT: On the day of your initial consultation appointment, you must bring with you:
- copies of any ultrasound scans, CT scans, MRI scans, Doppler tests or other X-rays that have been performed for the vascular problem for which you have been referred;
- a list of your current medications at your consultation
