This is an interagency collaboration between Shriners Hospitals for Children - Canada, the Montreal Children's Hospital, CHU Sainte-Justine and its Marie-Enfant Rehabilitation Center meant to improve the quality of life of patients who are suffering from musculoskeletal disorders that require complex care, and their families.
First of its kind in Canada, this initiative is based on these institutions, desire to join forces in making a difference to patients and families. The challenge is to transform the care and services provided, and to ensure access for the right patients, in the right institutions, at the right time, for the best care.
This job opportunity is a renewable, full-time, non-unionized, probationary term contract. Compensation is determined on the basis of recognized relevant experience. The other conditions are established according to the standards in force in the establishment. The incumbent will be expected for travel between the Initiative's partner institutions.

Job Description:
The synergy created by this global and collaborative Initiative aims to improve the management, treatment and rehabilitation of patients who attend many of these institutions. Concrete and meaningful solutions have been defined to address the issues faced by patients and families targeted by this Initiative. Among these solutions: the establishment of a patient navigator.
Under the leadership of the Initiative, overseen by the Shriners Nursing and Patient Services and the CHUSJ Rehabilitation Directorates, and attached to the Nursing and Patient Services at Shriners Hospital, the patient navigator will be mandated to accompany patients and families in their care and service trajectory and work closely with the resources of each institution.
More specifically, he will have to:
- Act as a resource person for patients and their families;
- Inform patients and their families of the organization of care and services available within the partner institutions of the Initiative; to do so, he will need to be present at various medical specialty clinics;
- Guide patients and their families to the most appropriate resources for their needs;
- Provide support to patients and their families, and listen to them to meet their needs;
- Promote the maintenance and / or social reintegration of patients by linking to appropriate community resources for the patient;
- Promote the education and training of patients and their families to enhance their ability to make informed decisions;
- Identify available resources in the Initiative's partner institutions and maintain ongoing communication with the identified resource persons;
- Plan and ensure coordination and continuity of care and services between the Initiative's partner institutions and all stakeholders, and strengthen interdisciplinary communication;
- Plan, implement and monitor individualized inter-institutional response plans;
- In collaboration with the nursing teams of the Initiative's partner institutions, contribute to improving the practices and tools in place that will allow greater fluidity in the inter-institutional trajectory;
- Collaborate on research projects and works and reflections aimed at achieving best practice for the benefit of patients and their families;
- Become actively involved in the Initiative's working groups aimed at improving interinstitutional care and services.
- Act in inter-facility problem-solving mode with the patient.
Requirements:
- University education, with clinical expertise in complex musculoskeletal care
- Minimum of (2) years of medical / surgical experience, including 1 year in complex musculoskeletal care
- Member in good standing of a professional order
