Complete set of forms

Dear Investigator/Coordinator,

Please note that as of April 4, 2011, all applications to conduct health research at the MUHC must be submitted through the new electronic online eReviews application.

To facilitate the transition to eReviews, the RI MUHC will continue to accept the downloaded application forms submitted to newstudy [at] muhc [dot] mcgill [dot] ca until May 15, 2011. However, it is recommended to take advantage of the transition period and become more familiar with the new system by viewing the training video and requesting a password to login to eReviews.
