How might incomplete information about your medications lead to complications? Does the frequent transfer of patients between care settings and the increasing number of medications taken by patients cause adverse drug events? Can electronic tools prevent such errors?

The McGill University Health Centre (MUHC) invites you to a second Café Scientifique to discuss the issues surrounding management of medications from hospital to home. The discussion will take place this Thursday, June, 14th 2012 at 5:45 p.m. in the relaxed atmosphere of Java U.
